What paperwork does slps need from the doctor?

SLPS will need a signed order form from your child's primary care provider, but this is something we can have them send us.

 

What should I bring to the evaluation?

For the evaluation you should bring ALL forms of insurance and your form of payment. If your child has an IEP, or paperwork relating to any psychological or developmental testing, please bring that with you as well.

 

What should I bring to the feeding/swallowing evaluation?

For feeding/swallowing evaluations please bring ALL forms of insurance a form of payment as well as the following:  what your child LIKES and DISLIKES to eat; what your child LIKES and DISLIKES to drink; what your child eats/drinks with, a plate, spoon, fork, cup, sippy cup, etc.

 

How long will the evaluation take?

Evaluation times truly depend on the child. However, most evaluations last between 1 1/2 hours to 2hours.

 

How long are appointments?

Appointments range from 30-60 minutes. 

 

How Often are appointments?

Appointment frequency depends on your child's severity. Appointments are usually scheduled for 1 to 2 times a week.

 

Will you remind me about our appointment?

Yes, we send out reminder texts  the business day before your scheduled appointment. These texts are also a means of confirming your appointment. If you choose to opt out of this service then we will simply confirm over the phone.
