First Steps
If you are concerned about you or your child’s speech progression and/or swallowing skills, the last thing you should worry about are the details involved in the referral process. Please see below our easy step-by-step list of how to secure a visit in our office.
To qualify for the first evaluation, you will need to be sure that you or your child has been seen by his/her primary care provider within the last year.
Step 1: The patient's medical provider will fax over the referral paperwork requesting an evaluation and therapy (if needed).
Step 2: We will then send our provider order form to the listed primary care physician.
Step 3: Once the necessary paperwork has been received, our office will call you to schedule an appointment at your earliest convenience.
Step 4: On the day of your appointment, you will need to bring you or your child’s insurance card or proof of insurance, as well as a payment card to add on file. Please know that we accept only cash and/or card.
Step 5 (optional): To get a headstart before the evaluation day, you can download our initial paperwork. Fill it out and email it to us before the appointment!
